The Center for End of Life Transitions
North Carolina Health Care Power of Attorney, North Carolina Living Will,
5-Wishes Document, and other End Of Life Documents
Call to set up a workshop for your community.
This workshop is a total of 8 hours, held over four two-hour meetings.
By settling, as much as possible, the matters of your world and making decisions (cremation, burial, life support, organ donation, etc.) ahead of time, you create an opportunity to enter your end of life, the moment of death and what may follow, with greater awareness and freedom. Perhaps you have thought about starting, completing, reviewing, or even finding your end of life documents. We offer you an opportunity to participate in a casual group setting where we will work to complete these documents. The goal: at the end of our last meeting you will walk away with a notarized set of end of life documents and know what your next steps are.
We will openly discuss our spiritual beliefs, desires, concerns and fears around the choices that could be made at a time when we are not able to speak for ourselves. You will choose your health care agent, consider organ/tissue/whole body donation, and begin to think about your after death care. We will explore Aging With Dignity’s Five Wishes Document. This is a North Carolina legally accepted document which guides you through the process of letting your family and doctors know who you want to make health care decisions for you, the kind of treatment you want and don’t want, what comfort care means to you, how you want to be treated, and what you want your loved ones to know.
Our guide for this workshop, Caroline Yongue, Minister, End-of-Life Educator, and Home Funeral Educator and Guide, is committed to helping individuals realize that they have options at the end of life, options which go beyond their last breath.
The fee for this four-class workshop (8 hours total) is $150. Checks are to be made payable to: ASM – CEOLT. Mail your payment to: Caroline Yongue, CEOLT, 171 Montford Avenue, Asheville, NC, 28801.
This workshop is the first in a three part series. Completion of this workshop, allows you to continue your end-of-life preparation by participating in:

End of Life Initiatives
Through the completion of our end of life documents, contemplating and envisioning our own “good death”, and making decisions about our after-death care, we lessen fear about our final days and also offer a gift to our loved ones. We leave our loved ones with the information they need to fulfill our wishes, thus relieving them of the tension and stress that may arise when trying to guess what we would have wanted. By being prepared and informed of our options, we are able to step into this end of life transition with compassion, patience and awareness, whether it is the transition of our own body or that of a loved one.
